The Hidden Physical Cost of Studying: Why Students Get Neck, Back and Forearm Pain — and What Actually Helps
A ten-year-old hunched over a tablet and a twenty-year-old grinding through a term paper at 1 a.m. have more in common than either would admit. Both are holding the same position for hours: head forward, shoulders rounded, low back slumped, hands locked over a keyboard. And both are far more likely than they realize to end up with a stiff neck, an aching low back, or a wrist that burns after a long typing session.
So can studying really cause physical pain? Yes — and the research is fairly consistent about it. A systematic review of university students found neck pain affecting roughly 46% of them, and one large study of over a thousand undergraduates found that close to 60% had experienced neck pain at some point, with about 17% dealing with it right now. Among younger students, non-specific low back pain has been reported in more than 40% of schoolchildren in a study of over 11,000 kids. The cause usually isn’t a single injury. It’s the accumulation of hours — a posture held too long, too often, by a body that isn’t getting much else during the week.
The good news is that this is one of the more manageable problems in musculoskeletal health. Below is what’s actually happening in the neck, low back and forearms during a study session, why regular exercise through the week matters more than any single stretch, and when it makes sense to have a physiotherapist take a look.
Three Areas That Take the Load
The neck
The average adult head weighs somewhere in the range of 4.5 to 5.5 kilograms — roughly a bowling ball. When it sits balanced over the shoulders, the muscles at the back of the neck do very little work. When it drifts forward to look at a laptop screen, a textbook or a phone, those muscles have to hold that weight against gravity, and they don’t get a break until the position changes.
This pattern has become common enough that researchers have given it a name: text neck. In one study of nursing students who shifted to online learning, reported neck pain rose to nearly 63%, roughly double the rate before the switch. What students usually notice isn’t sharp pain at first. It’s a dull ache between the shoulder blades, tightness at the base of the skull, and a neck that feels stiff when turning to check a blind spot while driving.
The low back
Sitting is not inherently harmful. Sitting in one position for hours is where the trouble starts. In a slouched seated posture, the pelvis rolls backward, the natural inward curve of the low back flattens, and load shifts onto the discs and the ligaments at the back of the spine rather than being shared with the muscles. Hold that for a three-hour lecture or a long study block and those tissues begin to complain.
For younger students, backpacks add to the picture. Most guidance suggests keeping a school bag under about 10–15% of a child’s body weight, and yet one study of schoolchildren found between 66% and 80% were carrying bags above that 10% threshold. Interestingly, the research on bag weight is more nuanced than the headlines suggest — one large study found that how heavy a bag felt to the student was a stronger predictor of back pain than what it actually weighed, which points back at overall conditioning and tolerance rather than the bag alone.
The forearms and wrists
This one is the most commonly overlooked. Typing looks effortless, but it involves thousands of small, repetitive finger movements while the wrist is held in a fixed, slightly extended position and the forearm muscles maintain a low-level contraction the entire time. Add mouse or trackpad use, add phone scrolling between classes, and the same tissues rarely get a full rest.
In a follow-up study of nearly 7,000 computer users, around 10% reported significant elbow, forearm or wrist and hand trouble over a twelve-month period. Students typically describe an ache along the top of the forearm, a tender spot near the elbow, or a heaviness in the hand after a long assignment. These symptoms usually settle with a change in load and some targeted strengthening, particularly when they’re addressed early.

Why Exercise During the Week Is the Real Answer
Here’s the part that surprises most students: the fix for pain that comes from sitting is rarely found in the sitting itself. Better chairs and monitor risers help, but they don’t build capacity. Exercise does.
Muscles that are stronger and better conditioned tolerate sustained positions for longer before they fatigue and start sending pain signals. This is why a student who trains a few times a week can sit through a three-hour exam without much complaint, while a classmate with the identical desk setup is squirming by the second hour.
The research supports this fairly directly. A systematic review of workplace interventions found moderate-quality evidence that neck and shoulder strengthening exercises and general fitness training reduced neck pain in people who were already symptomatic, with strengthening showing the larger effect. In a 20-week randomized trial of 351 office workers, groups doing neck and shoulder strength training three times per week reported reductions in both neck pain intensity and headache intensity compared with a group that did no training.
Canada’s own movement guidelines set a reasonable target. Children and youth aged 5–17 are advised to accumulate at least 60 minutes of moderate-to-vigorous activity per day, plus muscle and bone strengthening activities on at least three days a week. Adults 18 and over — which covers most university students — are advised to get at least 150 minutes of moderate-to-vigorous aerobic activity per week, plus muscle strengthening on at least two days.
Most students fall well short. ParticipACTION’s most recent Report Card on Physical Activity for Children and Youth found that only 37% of Canadian children and youth aged five to 17 met that daily 60-minute recommendation. Among university students, long study blocks, commuting and part-time work tend to squeeze activity out of the schedule at exactly the point when academic demands are highest.
A Realistic Weekly Plan for a Student Schedule
None of this requires a gym membership or an hour a day. It requires consistency.
Break up the sitting. Standing up and moving for even one or two minutes every 30 to 45 minutes changes the load on the spine and gives the forearm muscles a genuine rest. Setting a timer during study blocks works better than relying on memory.
Strengthen twice a week. Two sessions of 20 to 30 minutes covering the upper back, shoulders, core and legs is enough to make a meaningful difference for most students. Rowing-type pulling movements are particularly useful for anyone who spends the day rounded forward over a screen.
Get the aerobic minutes in. Walking to class instead of driving, cycling, a recreational sports league, or a few runs a week all count. For younger students, unstructured active play and organized sport do the same job.
Give the forearms a break. Alternate typing with reading or handwriting, keep the wrists relatively neutral rather than bent back, and take a genuine pause from the phone during study breaks rather than swapping one screen for another.
Protect sleep. Sleep is when tissues recover. Chronically short sleep tends to lower pain tolerance, which is part of why pain often feels worse during exam periods.
Check the setup. Raise the laptop so the top of the screen is near eye level and use a separate keyboard, sit far enough back in the chair that the low back is supported, and keep the backpack on both shoulders with the straps snug.
When to Book an Appointment With a Physiotherapist
Most study-related aches settle on their own with movement and a change in habits — but there are clear signs it’s worth having someone assess it properly. Book an appointment if headaches are becoming a regular feature of your study weeks, particularly headaches that seem to start at the base of the skull or the neck and build through the day, since neck-related tension is a common contributor. Book if pain has been hanging around for more than two or three weeks, if it’s waking you at night, if it’s getting worse rather than better, or if you’re noticing numbness, tingling or weakness in an arm or hand. Book if pain is starting to affect how well you concentrate, sit through a class, or get through an assignment. And it’s also entirely reasonable to book simply because you know you should be exercising and haven’t been able to make it happen on your own — a physiotherapist can assess where you’re actually loaded and vulnerable, then build a short, specific program that fits a student schedule rather than a generic list of exercises. For a lot of students, having a plan built around their timetable, their symptoms and their term is the difference between intending to exercise and actually doing it.

Should I stop typing if my wrist or forearm hurts? Not necessarily, but the load usually needs to be modified. Shortening typing sessions, adjusting wrist position and adding specific strengthening are often more practical than stopping altogether. If symptoms include numbness, tingling or grip weakness, have it assessed.
Can physiotherapy help with headaches? Some headaches have a neck-related component, and assessment can help determine whether that’s a factor in your case. A physiotherapist will look at neck mobility, muscle tenderness and how your posture and activity levels fit into the pattern, then build a treatment plan around what they find. Headaches have many possible causes, so an assessment is the right starting point rather than an assumption.
